Yizkor Book Order Form

Honor the memory of your loved ones through Tzedaka and by inscribing their names in our Annual Yizkor Booklet. 
The Yizkor book will be reprinted prior to Yom Kippur and will be used throughout the year.

The deadline to submit names is September 24th. 
 Please submit your form and payment no later than that date.

If you log in now, (click login in the upper right hand corner*) the following information will automatically populate, your contribution will immediately be attributed to your account and you will be able to pay by check (choose “Bill to my account”) to save yourself the credit card convenience fee. 

* if you have not logged in before or forgot your password, click “Forgot password” and you will receive an email to reset it. 
 
To view the ads from last year, you can click here which will open up a pdf version of the Yizkor book.  You will need to scroll through to find your ad.
Also, you still must select the size of the ad or the number of lines to be included in the Yizkor book, so that the correct amount can be calculated.
 

Please select the size of the ad; Full page, up to 25 names ($450), Half page, up to 15 names ($250) or Quarter page, 4 names maximum ($150).
For individual listing of name ($18 each) to be included on the Memorial page, please indicate the number of people and enter the names below.
 
Members can pay by check if they would prefer.
If you are not logged in, please pay by credit card.  A service fee will be added to cover the credit card fee.
If you are paying by check, please send in your payment soon.

Names will only be included after payment is received.
